 1) All Medical HBOT requires a physician prescription, eligible patients may obtain from one their doctor.

2) All transactions are subject to the 3.5% merchant credit card processing fee. No fee for check or cash.

3) Any treatment session cancellations or reschedules require TWO business days' notice. Failure to provide adequate notice may result in forfeiture of the scheduled session.

4) Packages must be used within the following timeframes: 15 days (5 treatment packages) 30 days (10 treatments), 60 days (20 treatments) from the date of first treatment. Packages must be started within (90 days) of purchase.

5) No refunds after packages are 50% or more complete. Any eligible package refunds will forfeit the package discount. No credit card fee refunds, and any refunds subject to processing fee.

6) All discounted pricing is subject to specific patient qualifications. Eligibility is verified during a complimentary case management phone call.

7) The discounted rate is a cash rate only and health claim form documentation is NOT provided for insurance submission. However, HBOT packages are HSA/FSA eligible and receipt documentation is provided.

Patient understands submitting documents to insurance company seeking reimbursement for services received at the discounted cash rate, will void the discounted cash rate and patient will be responsible to instead pay the insurance stipulated patient responsibility amount.

8) Additional Charges: Medical Air Brakes are $50 per session for up to 3 air brakes.
