1) All Medical grade Hyperbaric Oxygen Therapy (HBOT) requires a physician prescription. For self-pay patients seeking HBOT for an off-label medical condition, the cost for the physician consult is $300 and requires a non-refundable $50 consult deposit at time of scheduling.  This includes medical clearance, physical exam and HBOT prescription. 

2) All transactions are subject to 3.5% merchant credit/debit card processing fee. No fee for check or cash. 

3) Any cancellations or reschedules require TWO business days notice. Failure to provide adequate notice may result in forfeiture of the scheduled session or a doctors visit cancellation fee. 

4) Packages to be used within the following timeframes: 30 days (5 treatment packages) 60 days (10 treatments), 120 days (20 treatments) from the date of first treatment. Packages must be started within (30 days) of purchase. After 6 months, any unused sessions are billed at the current Single Treatment price.  

5) Any eligible package refunds will forfeit the package discount. The Single Treatment rate will be applied on used sessions. No credit card fee refunds, and subject to processing fee. Unused sessions/credit expires 1 year after purchase. Treatments/credits are transferable within our clinic to another patient with a valid prescription.   

6) Your first HBOT treatment can be scheduled directly after the doctor’s consult, pending clearance. A deposit of $250 is due at time of scheduling, which is refundable if not medically cleared. Deposits are non-refundable if appointment is cancelled, or if not rescheduled at least 2 business days prior to appointment.

7) ONE Single Treatment payment amount may be applied towards a package to receive the package discount. The remainder of package payment is due on second treatment date, and/or within 3 business days of first treatment.

8) Discounted pricing is subject to specific patient qualifications. Eligibility is verified during a complimentary case management phone call.  

9) Medical HBOT can be HSA/FSA eligible and receipt documentation is provided. Coded visit reports are available to the patient, upon request. We do not submit medical records to insurance for off-label indications.  

10) Individual Maintenance Treatments are $50 off the Single Treatment price, for established patients who have completed a treatment package. Custom Maintenance Packages are available upon request.
